The following documentation is required before you can be admitted as a student of Blackfeet Community College .  Please check off as completed, and return completed forms to Blackfeet Community College Student Services Department.

 


         Application for Admissions (attached)- Please print in legible black or blue ink, you must indicate a Degree Program.  If you have attended BCC in prior years, please indicate, as a former student and list the dates of attendance.  Please fill out form completely, sign, date and return to the Student Services Department.

Immunization Record-Verification of measles, mumps, and rubella #2 shots; (MMR #2).  These can be obtained at Indian Health Services for those enrolled in a federally recognized tribe.  Disregard if you were born before 1957.

Tribal Enrollment Certificate-If you are an enrolled member of a federally recognized tribe please submit certification, no photo ID’s, only certificates will be accepted.  (Blackfeet Tribally Enrolled Members can pick up their Tribal Enrollment Certificate from the Blackfeet Tribe, Enrollment Department.) All others may need to contact their Agency Enrollment Department to obtain a certificate.  Non-Native Americans please disregard this requirement.

Social Security Card-A copy of a social security card must be submitted for acceptance as a student to BCC.  This must be included to complete your student record for admissions and financial aid.

 

Verification of High School Diploma-A copy of a high school diploma must be submitted for acceptance as a student to BCC.  This is a requirement for all students, regardless of attendance at other colleges/universities, or graduate of other institutions.  _____Month _____Year          

 

GED Certificate-A copy of a GED certificate must be submitted for acceptance as a student to BCC.  This is a requirement for all students, regardless of attendance at other colleges/universities, or graduate of other institutions.

Ability to Benefit-A copy of scores from the TABE test, along with a letter stating that the student has passed the assessment/placement test, must be submitted for acceptance to BCC. 

           

Transcripts from other Colleges/Universities-If you have attended other universities or colleges other than Blackfeet Community College, please have an official copy sent to the Registrar at BCC, this must be included in order to complete your student record for admissions.
